The frequency of centrally generated spontaneous efferent activity recorded from the contralateral saphenous nerve was significantly higher in rats with CFA-induced inflammation 2.9 1Hz, n 16 ; than that seen in control non-injected rats 0.1 0.05Hz, n 5; p 0.01, Mann Whitney ; . Following removal of knee joints and formamide extraction, Evan's Blue dye extravasation measured spectrophotomically ; was significantly higher in contralateral knee joints of CFA rats than in the knee joints of control rats p 0.01, 1-way ANOVA ; . These data indicate that vascular changes had occurred in the uninvolved joint, contralateral to the arthritis. Here we report that a unilateral arthritis results in the generation of contralateral spontaneous antidromic activity in capsaicin-sensitive sensory nerve fibres. In addition, vascular changes dilatation increased permeability ; were found in knee joints contralateral to arthritis when no overt contralateral inflammation was present. In conclusion, increases in antidromic activity in sensory nerve fibres and vascular changes occur contralateral to CFA-induced arthritis. Two studies presented by Dr. Dean Andropoulos from Texas Children's Hospital evaluated pre- and post-operative cerebral oxyhemoglobin saturation in neonates undergoing cardiac surgery using cardiopulmonary bypass. Interestingly, the mean regional cerebral oxygen Shobha Malviya, MD saturation index values rSO2i ; were higher preoperatively and lower postoperatively in infants with single ventricle compared to those with two ventricles. Additionally, the majority of the single ventricle patients had rSO2i values above the threshold associated with neurologic dysfunction of 40-50% preoperatively while the majority of these values were below this threshold postoperatively. The converse was true for the two ventricle patients. While only one patient had clinical evidence of neurological changes, further follow-up of the entire group is currently underway. Another interesting study by Dr. Joe Tobias from the University of Missouri compared transcutaneous CO2 levels above and below the aortic cross clamp in children undergoing aortic arch surgery. He reported significant increases in TC-CO2 below the clamp with no changes in these values above the cross clamp. Furthermore, the degree of metabolic acidosis correlated with the lower TC-CO2 increase. Tobias speculated that the degree of tissue hypercarbia in the lower extremity may differentiate patients with adequate collateral circulation from those with poorly developed collateral circulation and may identify patients at risk for spinal cord ischemia. Dr. Priti Dalal from St Louis Children's Hospital presented a study addressing the controversial subject of propofol sedation by non-anesthesiologists. In this study of children who received propofol, pentobarbital and chloral hydrate for MRI, 2.7% of those who received pentobarbital and 11% of those who received propofol experienced severe oxygen desaturation. While children who received propofol had faster induction and recovery times, the net time saving was 15 minutes in the propofol group. Another study compared the efficacy of placebo vs two doses of midazolam administered intranasally in preventing emergence agitation following myringotomy and tube placement. In order to characterize and quantitate these complications, the charts of 157 patients with head trauma or intracranial bleeds with respiratory failure requiring mechanical ventilation between 1982 and 1987 were reviewed for evidence of segmental atelectasis and infiltrate on cheat roentgenogram reports. Of these, 38 patients having existing chest trauma on admission were excluded. Of the remaining 11 patients, pentobarbital coma was used to decrease the intracranial pressures in 109 patients. NMDA receptor antagonists such as PCP and ketamine have psychotomimetic effects in humans. In rodents, they evoke characteristic behavioral changes such as head weaving, sniffing, and ataxia, which are considered to be equivalent to the psychotomimetic effects in humans. Activation of the MLDS and increase in dopamine release in the NAC play a crucial role in the behavioral stimulation induced by PCP and MK-801 9 ; . There are two reports of ketamineinduced increases in dopamine release in the NAC. Irifune et al. 19 ; reported that 30 mg kg of ketamine injected ip increased mouse locomotor activity in association with an increase in dopamine turnover in the NAC by 21% of control. However, they did not measure the time course of dopamine release but assayed the level of dopamine and its metabolites in brain homogenates at 20 minutes after the ketamine administration. Hancock and Stamford 20 ; demonstrated that ketamine stereospecifically increased dopamine efflux from brain slices of the rat NAC. Barbiturates are addictive in humans 11, 12 ; , and an animal model of tolerance, physical dependence, and withdrawal effects for barbiturates has been established 21 ; . In the present study, we demonstrated that pentobarbital significantly decreased dopamine release in the NAC. Our results are compatible with the finding that GABA containing interneurons within the VTA and long-loop GABA-ergic feedback projections cause tonic inhibition on the VTA dopamine neurons 6, 7 ; . Diazepam, which potentiates GABAA receptor activity in response to GABA, decreases dopamine release in the NAC 22 ; , although the diazepaminduced reinforcing properties, which are recognized as the basis of drug addiction, seem to be mediated via the MLDS 15 ; . Unfortunately, we can neither elucidate the meaning of the decrease in dopamine release in the NAC in response to pentobarbital nor explain the mechanism of addiction and psychotropic effects by barbiturates. However, because the neurons that receive the dopaminergic signal in the NAC innervate and interact with final target brain regions related to drug addiction, such as the cortex, hypothalamus, amygdala, and hippocampus 7 ; , barbiturates and benzodiazepines may act directly on those brain regions. Our data also demonstrated that pentobarbital completely inhibited the increase in ketamine-induced dopamine release. This inhibition may be a mechanism by which barbiturates prevent ketamine emergence reactions. However, if we had used smaller doses of pentobarbital and or larger doses of ketamine, the ketamine-induced increase in dopamine release in the NAC would not have been completely inhibited by pentobarbital because ketamine dose-dependently increased and pentobarbital dose-dependently decreased dopamine release in the NAC. Behet's disease is an inflammatory disorder of unknown cause. There is often involvement of the gastrointestinal system, the central nervous system and large vessels, which can be life-threatening. As well, ocular lesions can cause blindness. Mucocutaneous symptoms are self-limiting but more frequent. Almost all the patients have recurrent oral aphthous ulcers, and more than 70% of the patients have genital ulcers and skin symptoms, which include erythema nodosum, pseudofolliculitis, papulopustular lesions, acneiform nodules and a positive pathergy test. The pathergy test is felt to reflect cutaneous hypersensitivity. In general, topical treatment using corticosteroids is satisfactory for these mucocutaneous lesions unless eye and vital organs are involved.
