1. Heat oven to 350F. Grease and flour one 1--quart ovenproof bowl and one 8 inch round cake pan. In large bowl, combine all cake ingredients; beat at low speed until moistened. Beat 2 minutes at high speed. Pour 2 1 2 cups batter into greased and floured bowl to within 1 inch of top; pour remaining batter into round cake pan 2. Bake at 350F. until toothpick inserted in center comes out clean. Bake cake in pan for 35 to 40 minutes; bake bowl cake 45 to 50 minutes. Cool cakes in pan and bowl for 15 minutes. Invert cakes onto wire racks; cool 1 hour or until completely cooled. 3. In small bowl, tint 1 cup of the vanilla frosting with brown and yellow icing colors to make desired tan color; blend well. Place cake from pan on platter or foil-covered cardboard. Frost top lightly with tan frosting. 4. With serrated knife, trim top of bowl cake to level; invert onto cake from pan. Trim around edges of cake from pan to create slanted edge for rim of helmet. Frost with tan frosting, sloping frosting at bottom to make rim and shape of army helmet. 5. Divide remaining vanilla frosting evenly into 2 small bowls. Tint 1 bowl of frosting with black icing color; blend well. Tint other bowl of frosting with green and brown icing colors to make desired army--green color; blend well. 6. Using pastry bags with round decorator tips, outline black, green and brown using chocolate frosting ; camouflage shapes on helmet as shown in photo. Fill in centers of outlines with remaining frosting. 7. Decorate helmet with round licorice candies and licorice twist as shown in photo. Use string licorice to outline rim of helmet.

2, 1 patient with hypopharyngeal cancer developed an eating disorder, which required intravenous hyperalimentation IVH ; management, so that this was evaluated as grade 4 toxicity. For this reason, three patients were added. In one additional patient with hypopharyngeal cancer, grade 4 mucositis appeared. Grade 4 toxicity occurred in two of six patients, but another four patients had only grade 2 adverse event and did not show DLT. In three patients at level 3, one patient with laryngeal cancer had only grade 2 adverse events, but one patient with hypopharyngeal cancer developed grade 4 mucositis, and another patient with laryngeal cancer developed serious blood toxicity when the second dose of DOC was given and the radiation dose reached 40 Gy, and disseminated intravascular coagulation DIC ; turned into septicemia, requiring dialysis in the ICU. This patient was saved and in the 3 years since then, the primary lesion and cervical metastasis have been controlled and the patient's activities of daily living have not been compromised. Toxicities in the above two patients were judged to be DLT, and level 3 was set as the MTD. From these findings, RD was placed at level 2, which was 35 mg m 2. Table 2 shows the occurrence of toxicities at each level. Apart from the DIC in the patient with laryngeal cancer, grade 4 mucositis occurred in a patient with hypopharyngeal cancer with an irradiation field of 17 13 cm, and it seemed highly likely that the level 3 dose could be tolerated in laryngeal cancers that require only 6 8 narrow fields of irradiation. The present series of patients has been followed up for at least 3 years. The larynx was preserved in all patients and no recurrence was observed during follow-up and liothyronine.

The quality of sexuality education is still dependent upon the efforts and competence of individual providers, and in Francophone and Germanophone Communities, individual school policy determines prioritization of the subject. Efforts to evaluate the provision of sexuality education have been made by the National Inter-Ministerial Commission Report of young people's sexuality 2001 ; , which recommended improving sexuality education in and out of school; the inventory of sexuality education provision 2003 and the Marchal Report 2004 ; , which recommended promoting sexuality education and lomotil.

Although surgical gastrostomy has been the standard technique for years, there is general consensus that, when possible, this technique should be replaced by percutaneous gastrostomy. In this study, fluoroscopically guided gastrostomy was clearly superior to percutaneous endoscopic gastrostomy in terms of feasibility. Of the requested gastrostomies, 99% were performed with fluoroscopically guided gastrostomy in a population in which percutaneous endoscopic gastrostomy had failed or was contraindicated in approximately 25% of patients. Passing the endoscope through a tightly stenotic upper digestive tract is a major drawback with percutaneous endoscopic gastrostomy, and this difficulty was exacerbated in this series of cancer patients. Use of a small-diameter catheter and a hydrophilic guide wire, however, allows this obstacle to be surmounted by penetrating the stenosis, thus making the procedure possible. Moreover, when necessary, gastric puncture with a Chiba needle can help overcome the rare failures of upper digestive tract catheterization. When air is already present in the stomach, it can be directly targeted at gastric puncture. When no air is seen at fluoroscopy, effervescent powder can prove useful, as in our experience. This method, which to our knowledge has not been reported earlier, avoids the need for gastric puncture under computed tomographic guidance, as reported by other authors in difficult cases 5 ; . The need for gastropexy is controversial in the literature because T-fasteners are considered capable of inducing considerable tension on the gastric wall. This tension in turn causes skin ischemia, which leads to parietal necrosis and finally enlargement of the gastrocutaneous tract. Fluid leakage then ensues around the tube 6 ; . The risk of hemorrhage is theoretically increased, as is the number of punctures. The low rate of hemorrhage in our series 0.2% ; and the potential benefit of tamponade induced by gastropexy 7 ; , together with the low frequency of enlargement of the gastrocutaneous tract in our series 1.2% ; , prompted us to recommend the use of T-fasteners. The advantages of gastropexy are that the gastrocutaneous tract matures rapidly, peritonitis and intraperitoneal tube replacement are avoided, and there is thus no need for a new fluoroscopically guided gastrostomy in case of tube dislodgment.
