From 10% to 46% of patients with HIV infection in clinical cohorts from Europe and USA are coinfected with hepatitis C virus HCV ; . HIV HCV coinfection is particularly frequent in some areas. Thus, in Southern and Eastern Europe about one half of HIV carriers are also infected with HCV. Coinfection involves mostly intravenous drug users who have shared needles, since both viruses are blood-borne agents. In addition, acute HCV infection cases are increasingly seen in HIV-positive men who have sex with men. The frequency of HCV-related chronic disease as a cause of morbidity and mortality in coinfected patients has increased in the last decade. Currently, end stage liver disease ESLD ; is a leading cause of death in this population. HCV and HIV may interact with each other. Whereas the impact of HCV coinfection on HIV disease progression has been a matter of controversy, coinfected patients seem to show a blunted CD4 cell response after starting antiretroviral therapy ART ; . On the other hand, HIV accelerates the progression of HCV-related liver disease. Thus, coinfected patients with acute hepatitis C develop chronic liver disease more frequently than monoinfected patients. Likewise, HIV shortens the time to the development of cirrhosis, ESLD, hepatocellular carcinoma and death due to liver failure. Risk factors for liver disease progression include older age of HCV infection, lower CD4 cell counts, alcohol consumption and lack of potent ART. Antiviral therapy against HCV also slows down the progression of liver disease. In patients with ESLD, factors related with the severity of liver damage, such as Child-Pugh-Turcotte score or the presence of hepatic encephalopathy are also predictors of death. Anti-HCV therapy and effective ART are the most useful tools currently available to prevent rapid liver disease progression in HCV coinfected patients. In patients with ESLD, liver transplant should be considered early. Not surprisingly, the first nations that implemented together these ideas, constitute after the cold war the most developed societies of the world. There is a non-declared principle that imposes an obligation upon the rest of the peoples to adapt their societies to free market and respect for human rights models. The end of the cold war and the following speed up of globalisation constitute a revolution of Copernican magnitude. This change could be lead solely by Capitalism in a single dominant power system or could be lead by an international organisation that adds a liberal - democratic dimension to classic capitalist rule. The main argument of this paper will try to demonstrate that the tensions that presently exist between the values of liberal Democracy and classic Capitalism are expressed by the causes that lay behind the Iraqi war, deciding the future of the world decision making system. Northeast Asian FTA Dogged Down and Seeking its Breakthrough Zhang Xizhen Peking University, China According to general integration theories, China, South Korea and Japan have many favorable advantages to realize regional integration. Firstly, geographically the three countries are all located very close, thus the cost of trade and personal exchange can be minimized. Secondly the three countries' economies have complementarities. They are at various levels of industrial structure and their major export goods have no conflicts. Thirdly the three countries share some traditional cultural elements. Confucianism is widely accepted by most people in China, Korea and Japan. But unfortunately these advantages have not promoted regional integration and establishment of FTA in Northeast Asia. This paper would like to examine factors that impede the regional integration and establishment of Northeast Asian FTA. There are four major obstacles. First one is protectionist policy on domestic markets. As far as these three countries are concerned, it is Japan that is adopting the most severe protectionism, especially on its agricultural products against the same goods from China and other countries. Second obstacle is deep skepticism between the countries. Due to Japanese invasion and colonial domination in China and on Korean Peninsula in history, there still remains deep skepticism in China and South Korea toward Japan. Third obstacle is the leadership issue in forthcoming Northeast Asian FTA. International reality is that the biggest and strongest countries dominate and play leading role in any international community. In fact there is a very contradictory phenomenon that on one hand China and Japan don't want to openly take leadership position, and on the other hand they respectively promote FTA or free trade relationship with ASEAN countries. This is seen that they are fighting for the leadership. Fourth obstacle is the U.S. interference.
