7.2.1 Guidelines for transition to the future communications systems should be such as to encourage early equipage by users through the earliest possible accrual of the systems benefits. While pemphigus itself may cause eosinophilia, an increase in eosinophils during gold therapy has been reported to herald other complications and should lead to withholding the use of gold until it resolves.13 Cutaneous adverse effects, such as pruritus and drug eruptions, are common, as are proteinuria and nitritoid reactions.14 Gold therapy has been restarted after minor toxic effects such as dermatitis and mild proteinuria resolve, without a recurrence of adverse effects.1, 14 A recent report11 recommends a combined dermatologist and rheumatologist approach to the treatment of pemphigus with gold. While all patients receiving high doses of corticosteroids should be followed up by a primary care physician to monitor cardiac, pulmonary, and renal status, we have not found it necessary to consult with rheumatologists are well versed in the treatment of rheumatoid arthritis with gold and may be helpful in assisting dermatologists when initiating gold therapy; however, the dermatologist will ultimately make most therapeutic decisions in An often-cited criticism of studies7, 12 investigating treatment regimens for pemphigus is that steroid-sparing effects are difficult to claim without adequately controlled trials. In all previous studies, historical controls were used. Indeed, only 4 of our patients were treated with gold alone.
Repeat renal biopsies had been performed after more than 8 months of observation. The average interval between the first and second biopsy was 4331 months. The mean number of glomeruli examined at the first and second biopsy was 4325 and 5538, respectively. Of the 21 patients with WHO class IV at the first biopsy, one showed conversion to class I normal ; and two to class III focal proliferative lupus nephropathy ; at the second biopsy. All three had a glomerular activity index of 1 + 1.5 + at the time of the first biopsy. Of the 21 patients, seven 33% ; showed deterioration of renal function at the time of the second biopsy. None exhibited transformation of WHO class. The seven patients with clinical progression also showed a significantly higher frequency of hypertension, higher per cent glomerulosclerosis and larger interstitial volume density at the time of the second biopsy as compared with the 14 patients without progression Table 1 ; . CI Pages 242-243 Page 33 of 44 Title of element unit 05211 MLA 1: Promoting press freedom, freedom of expression and democracy Inputs Funding Regular budget , 827, 900; Extrabudgetary Justification Identification of needs Background Freedom of expression is the cornerstone of any democratic society, which UNESCO, with its mandate to promote the "free flow of ideas by word and image", is called upon to foster. UNESCO's action in this field places emphasis on creating an environment that is conducive to the full enjoyment of freedom of expression, by promoting press freedom as well as enhancing the importance of freedom of expression in democracy among decision-makers and civil society. UNESCO's action in promoting freedom of expression and its corollary, the freedom of the press, is of particular relevance in the development of knowledge societies. The new media landscape generated by the rapid development and spread of information and communication technologies poses complex challenges which require new approaches to ensure freedom of expression, access for all, and the free flow of information and knowledge. Strategies Implementation The main strategy, which aims at promoting and upholding the principle of freedom of expression, as defined in Article 19 of the Universal Declaration on Human Rights, consists of five interrelated actions. The first action will be the awarding of the UNESCO Guillermo Cano World Press Freedom Prize, which has become an integral part of each year's celebrations of World Press Freedom Day 3 May ; and concretely expresses UNESCO's commitment to freedom of speech and to those who suffer from repression and persecution in the exercise of their profession as journalists. Secondly, UNESCO, with its partners, will play a leading role within the international community in sensitizing the public as well as governmental authorities to press freedom and in protecting the right of journalists through the International Freedom of Expression Exchange IFEX ; alert network, grouping 1, 500 members in more than 130 countries and regional networks. Thirdly, support will be provided to implement UNESCO's 29 C Resolution 29 on combating the impunity of violence against journalists and media institutions. Fourthly, support will be provided to media professional organizations to promote safety for local and international news reporters working in conflict areas. Fifthly, efforts will be deployed to raise awareness of the importance of privacy and freedom of access to information in libraries and other traditional information institutions. The Declarations adopted at the conferences have been disseminated in various forms worldwide. The Belgrade Declaration stresses that independent local news media are essential to provide trustworthy information that is vital for peace and reconciliation efforts. The Dakar Declaration emphasizes that independent and pluralistic media are essential for ensuring transparency, accountability and participation as fundamental elements of good governance and human rights. These Declarations were endorsed by Member States at the last General Conference. Results not achieved Lessons learned, success factors and challenges Amid growing recognition of the importance of press freedom for democracy and development, in 1993 the United Nations General Assembly proclaimed 3 May "World Press Freedom Day". Throughout the world, this Day was widely celebrated during the last two years and served as an occasion to raise awareness of violations of the right to freedom of expression and drew attention to the dangers and risks, including death, that media professionals face in the discharge of their duties and iloprost.

N preparing my thoughts and feelings in order to put them into words during this time of year, I find myself reflecting on priorities, traditions and faith. It is no surprise that the events of September 11th of this year have caused all of us to take stock of what is important in our lives. The opportunity during this time of year to be with our families, relatives and special friends is something we may have taken for granted in the past. This is no longer the case. The opportunity to live in a society where the faith or belief we choose to follow or practise is not persecuted, but rather is respected and enhances our ability to understand each other. As I traveled around our province recently for our district meetings, I was fortunate enough to meet with many of my peers. The beautiful scenery of the many parts of Saskatchewan I had not seen before was my visual Christmas card and the greetings and commitment to your profession were the words for that card. Ongoing support and training on ICT will be given to groups where possible. This will be carried out as one-time training sessions by experts as well as ongoing online support from partners. Support will be both technical in nature as well as supporting groups to use the Internet for campaigning and advocacy work; Group-to-group partnerships and training will be facilitated wherever possible to engage in expertise-sharing in ICT skills and advocacy. This will involve communication via email and telephone, and where possible visits from the organisation offering expertise to work one-on-one with staff at the group; Training provided by IFEX staff concerning how to use the Internet for Action Alert campaigning, and how to research and develop Alerts; Clearing House staff will provide groups with a reliable and affordable website host, particularly where the local costs are prohibitively expensive; The ICT Development Manual will be prepared by Clearing House staff and put online. Such a coupling pattern has been demonstrated previously for other primarily G, -coupled receptors summarized in Ref. 10 ; . In agreement with previous reports 30-32 ; , our immunoblotting experiments demonstrate the expression of G, - and G, -like G proteins in insect cells. Studies of Gi, O-coupled receptors in baculovirus-infected insect cells raised the possibility that only small amounts of G, -like proteins are expressed in Sf9 cells in relation to the amount of receptor protein that can be introduced 31, 33 ; , and expression of the human + c adrenergic receptor in Sf9 cells revealed that maximal functional effects were recorded 12 h before the peak expression of binding sites was reached 34 ; . The latter observation could be related to a decreased expression of signal transduction components, e.g. G proteins, due to viral infection of the host insect cell.
Expression. IFEX seeks to facilitate exchange of information in the community, and to promote and defend freedom of the press and of expression in a colordinated manner, through action alerts and joint campaigns. The IFEX Clearing House also seeks to strengthen communication capacities in the developing countries, so as to promote their participation. By encouraging organizations in developing countries to participate in the Clearing House network through the provision of hardware and training, IFEX, like UNESCO, seeks to build the communication of capacities of those countries. 21. Lysl46 of rabbit aldolase A [D-fructose-l, 6-bis phosphate ; : D-glyceraldehyde-3-phosphate lyase, EC 4.1.2.13] was changed to arginine by site-directed mutagenesis. The of the resulting mutant protein, K146R, was 500 times slower than wild-type in steady-state kinetic assays for both cleavage and condensation of fructose-l, 6-bis phosphate ; , while the Km for this substrate was unchanged. Analysis of the rate of formation of catalytic intermediates showed K146R was significantly different from the wild-type enzyme and other enzymes mutated at this site. Singleturnover experiments using acid precipitation to trap the Schiff base intermediate on the wild-type enzyme failed to show a build-up of this intermediate on K146R. However, K146R retained the ability to form the Schiff base intermediate as shown by the significant amounts of Schiff base intermediate trapped with NaBH4. In the single-turnover experiments it appeared that the Schiff base intermediate was converted to products more rapidly than it was produced. This suggested a maximal rate of Schiff base formation of 0.022 s"1, which was close to the value of kcat for this enzyme. This observation is strikingly different from the wild-type enzyme in which Schiff base formation is 100 times faster than k . For K146R it appears that steps up to and including Schiff base formation are rate limiting for the catalytic reaction. The carbanion intermediate derived from either substrate or product, and the equilibrium concentrations of covalent enzyme-substrate intermediates, were much lower on K146R than on the wild-type enzyme. The greater bulk of the guanidino moiety may destabilize the covalent enzyme-substrate intermediates, thereby slowing the rate of Schiff base formation such that it becomes rate limiting.
