DRUG UPDATE Update on Gefitinib IRESSA ; Restrictions This is a reminder that no new patients may begin treatment with gefitinib for non-small cell lung cancer NSCLC ; . In addition, all patients who are to continue on gefitinib therapy must be registered with the IRESSA Patient Registry by 31 December 2006 to obtain a continued supply of medication. This is an earlier than the previously communicated date i.e., 1 February 2007 ; . These new restrictions follow the notice by Health Canada in June this year on the use of gefitinib as third-line treatment for locally advanced or metastatic NSCLC. Briefly, gefitinib therapy may be continued in patients who are benefiting from it but no new patients should be prescribed gefitinib therapy for NSCLC. This regimen is the standard treatment for Burkitt's lymphoma and leukemia acute lymphoblastic leukemia [ALL-L3] ; . Both of these malignancies show over-expression of the surface antigen CD-20. In a study from the MD Anderson Centre by Thomas et al. New Adjuvant Protocol for Breast Cancer The Breast Tumour Group has introduced an adjuvant protocol involving the use of a short course of trastuzumab in women with HER-2 positive early breast cancer.
With chronic myelogenous leukemia were treated with hydroxyurea.2 refractory to previous treatment with busulfan and 10 were patients in whom the diagnosis had been determined recently. The diagnosis of chronic myelogenous leukemia was established by evaluation of the peripheral blood smear and bone marrow, and determination of the Philadelphia chromosome and leukocyte alkaline phosphatase. The dose of hydroxyurea was 50 mg. Kg. day in two divided doses until the leukocyte count was less than 10, 000 per cu.mm., and then 20 mg. Kg. day with appropriate adjustments according to the weekly white blood count. The investigation has been conducted over a five-year period. All the blood counts were carried out by the same technician Miss Kay Newton ; utilizing the same standard method for the collection of blood samples and the determinations.

There is increasing evidence that, as in normal haematopoiesis, the malignant growth of the leukaemic cell clone is maintained by only a small fraction of highly specialized stem cells. Unravelling the exact biology of these leukaemic stem cells as the disease-maintaining cells is essential for understanding the leukaemic process. Our research aim is to identify, isolate and characterise candidate leukaemic stem cells in the most common type of childhood leukaemia, i.e. acute lymphoblastic leukaemia ALL ; . Using modern multi-parameter 4- and 6-colour ; flow cytometry, candidate leukaemic stem cell populations are defined by patterns of cell surface markers reflecting the earliest stages of lymphoid differentiation and leukaemia-specific expression profiles ; . These candidate leukaemic stem cell populations are purified by cell sorting for further analysis. Molecular testing for the presence of leukaemia-associated chromosomal abnormalities provides information on whether these cells belong to the leukaemic clone and thus indirect information on the cell of origin in which the leukaemia arises. In the context of work by many other groups, these experiments have provided the basis for a model of a stem cell hierarchy in childhood leukaemia: while good prognosis ALL t 12; 21 ; only involves the CD19 + B lymphoid compartment Fig. 1: dark blue lightning bolt ; , certain types of high risk ALL t 9; 22 ; and t 4; 11.

To protect the health and safety of Canadians and facilitate the generation of scientific data, research-grade marijuana must be of a reliable and consistent chemical composition and potency and originate from a licit supplier. Illicit marijuana, like other "street drugs" is often mixed or adulterated with unknown, harmful substances.12 Some have suggested using embargoed, seized marijuana for medical research. The Commission of Narcotic Drugs General Assembly has resolved under the United Nations 1961 Convention that countries, including Canada, should refrain from the proliferation of supply sources and avoid unforeseen imbalances caused by sales of seized and confiscated drugs and products manufactured from such drugs. The long standing policy of the organization overlooking treaties compliance is that countries should not base a licit activity i.e., research ; upon an illicit source. Consequently, the idea of recycling marijuana seized in enforcement activities is not a viable option. Securing a licit source will ensure that the marijuana used in medical research is of an acceptable, standardized quality, free from fungi, molds, pesticides or other contaminants. Securing a Governmental Source of Marijuana Since a dependable source of research-grade marijuana is necessary to enable the conduct of clinical trials and given that NIDA may not entirely fill Canada's needs, Health Canada is taking steps to establish a government-controlled growing operation in Canada. The proposed supply of marijuana would be used solely for medical research purposes. A business plan is scheduled to be and ifosfamide.

One route that has not been travelled previously is that of protein kinase C inhibition. This pathway may be exploited to give potent inhibitors, such as the bryostatins, now in clinical trial. A summary is given of the current status of topoisomerase, focusing on recent clinical advances with camptothecin analogs based on connecting empiricism with concepts of drug selectivity. Modification of existing therapies based on the pursuit of leads arising from mechanistic studies is also being applied clinically on a wide scale. Greater understanding should follow from the studies of reversal of the multidrug resistant phenotype, on the use of hydroxyurea to reverse resistance mediated by extrachromosomal DNA, and on various aspects of the fluoropyrimidine pathways. Successful applications of chemotherapy to the treatment of specific diseases include the growing applications of systemic therapy using various skin malignancies. In prostate cancer, estramustine phosphate will likely play an expanding role. In addition to its inhibitory effect on the synthesis of DNA precursors 4 ; , hydroxyurea evidently also interfereswith glycerol oxidase. This enzyme occurs in the mycelia of several strains of Aspergillus, Neurospora, and Penicillium 5, 7, 8 ; . In the initial description of this enzyme 5 ; it was indicated that it could be inhibited by cyanide, hydroxylamine, or azide. We saw no inhibition of the glycerol oxidase-mediated assay for triglycerides by hydroxylamine or urea, even in concentrationsseveral-fold those at which hydroxyurea strongly inhibited the assay. The observed time course for inhibition of triglyceride determination in serum after a single oral dose of hydroxyurea Figure 1 ; is similar to that for actual hydroxysrea concentrations in serum 9 ; .
