The implications of our results, however, are most likely to be clinically relevant because a direct association between anatomic changes in coronary stenosis and future clinical events has been demonstrated by angiographic trials.29 In summary, the present study provides compelling evidence that in white, middle-aged men with established CAD, the HL gene 514 C3T polymorphism significantly predicts changes in coronary stenosis with lipid-lowering therapy. In addition, this study provides the pathophysiological mechanism to account for the effect of this genetic polymorphism on CAD response to treatment, highlighting how current routine lipid measurements may not enable physicians to distinguish between responders and nonresponders. Screening for these variants in the HL gene promoter region identifies CAD patients who will benefit most from lipidlowering strategies, as well as subjects who appear to be resistant to HL-mediated CAD regression. In these resistant patients, a more aggressive LDL-Ctargeted and overall risk-reducing approach might be warranted. The relevance of these findings is emphasized by the high frequency of this polymorphism, ranging from 20% to 47%, depending on the population studied. Therefore, screening for this genetic variant could become an important parameter influencing the choice of treatment strategies for cardiovascular risk reduction and their cost-effectiveness.

Laboratory would be undertaken by IHEP and other institutions in China. A European collaboration has proposed the Double-CHOOZ experiment. This proposal intends to make use of the existing underground laboratory near the Chooz nuclear power plant for the far detector and to build an additional near detector next to the power plant. The project received scientific approval in France in March 2004, and a collaboration consisting of groups in France, Italy, Germany, and Russia has been formed. Construction of the far detector can commence as early as 2006 assuming that the respective funding agencies in France and Germany will approve the proposal in 2004 2005. Together with EDF Electricite de France ; , the Double-Chooz collaboration is studying the possibility of building an underground covern at a depth of 8 m with additional overburden in form of a man-made mountain. The cost for this construction is likely to be covered by EDF. Without the civil construction cost for the near detector the Double-Chooz experiment is estimated at about Euro 10M. First results from both the near and far detector can be expected in 2008 2009. With this ambitious time scale the Double-Chooz experiment is likely to set the time scale for the worldwide search for 13 and cilium.

France System adequacy forecasts and effective operation showed that the generation load balance on the French system was not at risk during the winter. The real operating conditions were mostly better than expected, with the exception of the last weeks of January. Actual demand was lower than the forecasted level due to the mild temperature conditions during the winter. Generation overhauls were realised as planned. The general level of unplanned overhauls and outages conformed to forecasted values. Hydropower generation was higher than expected due to favourable climatic conditions. Wind generation is not yet significant for the generation-load balance in France. Two remarkable non predicted events occurred over the winter: - The outage of the Cordemais coal power-plants caused a risk of low voltage in Brittany from December 8th to the end of January; and - Low temperatures on January 25th caused the consumption level to nearly reach the record level. A windstorm that affected northern France on 8th December caused the tripping of 4 extra high voltage lines. The objective of this study was to detect whether there was any difference among the characteristics of patient satisfaction between two patient emphasis groups: patients demanding technical elements of hospital care and patients demanding interpersonal elements. Design and setting. The sample for this study was drawn from in-patients discharged from 77 voluntarily participating hospitals throughout Japan. The relationship between overall satisfaction with hospital care and patient satisfaction, and the evaluation of a hospital's reputation, was explored by stepwise multiple regression analysis of 33 variables relevant to aspects of hospital care for each patient group. Results. In the interpersonal emphasis IE ; group, `nurse's kindness and warmth' was associated significantly with overall satisfaction, while `skill of nursing care' and `nurse's explanation' were significant predictors of overall satisfaction in the technical emphasis TE ; group. On the other hand, `doctor's clinical competence', `recovery from distress and anxiety', and items pertaining to the hospital's reputation were significantly related to overall satisfaction in both emphasis groups. Conclusion. For overall patient satisfaction, it is essential to satisfy specific items related to the aspect of hospital care emphasized by the patient. Specific significant predictors of overall satisfaction e.g. `doctor's clinical competence' ; were indispensable measures of professional performance in hospital care, irrespective of the patients' emphasis. A positive perception of hospital reputation items might increase overall patient satisfaction with Japanese hospitals. However, studies have focused mostly on workplaces where a problem was already suspected. Local awareness may thus have affected subjects' health beliefs, favouring positive associations. Less opportunity for this bias exists in community surveys, as subjects are drawn from many different workplaces. A study was undertaken to assess the relationship between keyboard use and upper limb disorders ULDs ; in this setting. A questionnaire was mailed to the entire list of patients aged 2564 years registered with one general practice in Southampton n 3577 ; . Questions were asked about upper limb pain and use of keyboards for 01, 14 and 4 h average working day ; . Subjects with upper limb pain were asked to undergo a validated physical examination, and were classed as having a discrete ULD, or nonspecific neck or arm pain. Analysis was restricted to those in work. Associations with keyboard use in comparison with asymptomatic postal respondents ; were examined by log-logistic regression with adjustment for age and sex. Among 2036 respondents, 1534 held a job, comprising 716 subjects with pain in the upper limb and 818 others. Among the former group, 460 were seen and examined; 116 presented with discrete ULDs, 101 with neck pain and 131 with non-specific arm pain. Use of a keyboard for 14 h per day was associated significantly with neck pain.

A few case reports indicate that opioid analgesics can induce mania. The authors investigated the mood reaction of opioid analgesics in patients with bipolar disorder. Nine 27% ; of 33 patients who took opioid analgesics for medical reasons experienced a significant hypomanic manic reaction, and two other patients reported an antidepressant effect. None of the comparison subjects reported a significant mood reaction from opioid analgesics. These results indicate that opioid analgesics can have an important mood-altering effect on patients with known bipolar disorder.
An experimental program is conducted by the international collaboration DEAR at Laboratori Nazionali di Frascati LNF ; to measure the strong interaction induced shift and width of the kaonic hydrogen 1s atomic state via X-ray spectroscopy with unprecedented precision. The features of this experiment as well as the performance are described. Preliminary results of the analysis of the kaonic hydrogen X-ray data and a comparison with other experimental and theoretical results are presented. An outlook to the future and perspectives of the ongoing experimental program on kaonic atoms at LNF is given and clove.
