It was a great pleasure to become involved in the Skills at Work Programme through BITC. This important initiative gave us an opportunity to give something back to the local community. It proved to be a very positive experience and a learning curve in understanding the perspectives of young students who are starting out on their chosen career paths. Tracking ID # 172941 ; BACKGROUND: Recent reports from the Institute of Medicine emphasize patientcentered care and cross-cultural training as a means of improving the quality of medical care and eliminating racial and ethnic disparities. The objective of this study was to determine whether, controlling for training received in medical school or during residency, resident physician socio-cultural characteristics influence self-perceived preparedness and skill in delivering cross-cultural care. METHODS: We analyzed a national survey of 2047 residents in 7 specialties emergency medicine, family practice, internal medicine, obstetrics gynecology, pediatrics, psychiatry and general surgery ; in their final year of training at US academic health centers response rate of 60% ; . 9 resident characteristics were analyzed gender, importance to practice in a diverse patient mix, race, multilingualism, born in US, US medical school graduate, medical training or provision of medical care outside the US, specialty, and whether additional instruction in cross-cultural care was given during residency ; . We assessed: 1 ; self-reported preparedness to treat specific types of patients, manage specific issues and situations, or to provide certain services; and 2 ; self-assessment of skills in delivering cross-cultural care. Differences in preparedness and skill were assessed using the Chi squared statistic and multivariate logistic regression. RESULTS: Residents who felt it was important to practice in a setting that has a diverse racial and ethnic patient mix felt better prepared in five categories. Multilingualism is associated with being more prepared to deal with patients with limited English proficiency OR 1.76 ; and new immigrants OR 1.73 ; . African American OR 10.75 ; and Latino OR 5.0 ; resident physicians reported being very prepared to deal with racial or ethnic minorities. African American resident physicians were best prepared to deal with patients who had distrust in the healthcare system OR 2.29 ; . Asian resident physicians were more prepared to care for patients with health beliefs or practices at odds with Western medicine OR 2.0 ; . Any training outside the US confers more preparedness in dealing with new immigrants OR 1.61 ; . The most important factor associated with improved skill level in performing selected tasks or services believed to be useful in treating culturally diverse patients was having received skills training in residency ORs 2.805.60 ; . CONCLUSIONS: Our findings highlight the importance of resident physicians personal and cultural backgrounds in informing their sense of preparedness for crosscultural care. It underscores the need for including medical residents from diverse backgrounds in all training programs because of the invaluable personal experiences and charcoal.

APA Practice Guideline for Schizophrenia: Risperidone Equivalents TO THE EDITOR: Clinicians seek guidance regarding conversion factors that will allow them to compare novel neuroleptics to existing conventional agents in milligram equivalents. Recently published APA guidelines indicate that a 12-mg dose of risperidone equals a 2-mg dose of haloperidol 1 ; , suggesting that haloperidol may require doses twice as high as those of risperidone. However, several lines of investigation support equivalent dosing. Positron emission tomography findings indicate that risperidone and haloperidol are approximately equivalent in terms of dopamine 2 D2 ; occupancy. Specifically, mean D2 occupancies for risperidone and haloperidol at 2 mg are 66.3% and 66.7%, respectively 2, 3 ; . While it might be argued that risperidone's shared serotonin-dopamine antagonism prevents a comparison based simply on D2 occupancy, there is no evidence to date to suggest that risperidone is different from conventional neuroleptics such as haloperidol in terms of the need to achieve at least 60%70% D2 occupancy for antipsychotic response 25 ; . This is in contrast to clozapine, which clearly demonstrates its therapeutic effect at levels of D2 occupancy below 60% 5, 6 ; . Comparative data between risperidone and haloperidol, when flexible dosing is employed, provide further support for dose equivalence. In a recent double-blind study of firstepisode psychosis 7 ; , individuals were randomly assigned to 4 mg day of either haloperidol or risperidone. Thereafter, doses of each could be titrated by 2 mg, on the basis of clinical response and side effects, to a maximum of 16 mg day or a minimum of 2 mg day. Evidence involving patients with chronic schizophrenia, although more indirect, also suggests equivalent dosing. Unfortunately, in controlled trials directly comparing risperidone to haloperidol, lower doses of risperidone i.e., 116 mg day ; have been compared to haloperidol doses of 1020 mg day 810 ; . This has made it virtually impossible to establish whether these two compounds may be equipotent. However, other reports involving neuroleptics in lower doses, including a meta-analysis of 22 randomized, controlled trials, have indicated that for a compound such as haloperidol, the optimal daily dose range is between 3 and 8 mg 1113 ; . This closely parallels the multiple fixed-dose studies with risperidone indicating that 48 mg day appears to represent the optimal dose range for a similar patient population 810 ; . The recommendation that risperidone and haloperidol be considered equal in terms of dosing must be made with the caveat that these compounds are not necessarily clinically equivalent. Specific advantages have been linked to risperidone's concomitant 5-hydroxytryptamine antagonism, such 1301.

The calibration constants for the portable UAF calibrators were determined with reference to the measured sensitivities of ten different Chaparral Model 5 sensors. The microphone sensitivities had been determined previously at the Los Alamos National Laboratory LANL ; by Tom Sandoval and Rod Whitaker. The pressure tank in the infrasonic laboratory at LANL, operating at one hertz frequency with a peak-to-peak pressure of 3.22 pascals, was used as a standard reference to obtain the ten individual microphone sensitivities in units of volts per pascal. The ten Model 5 Chaparral sensors were then recalibrated using a one Hz calibration signal produced by the UAF calibrators. Using the individual sensor sensitivity, as determined at LANL, the average value of the calibrations constants for the UAF units were obtained. The calibration constant is a dimensionless number that depends on the ambient atmospheric pressure. When the value of the ambient pressure is multiplied by the calibrator calibration constant, one obtains the peak-to-peak calibration sinusoid signal amplitude. The average values of the calibration constants of the UAF calibrator units # 2 and # 3 were found to be: Unit # 2: Cal. Const. 1.328E -5 ; Unit # 3: Cal. Const. 1.331E -5 ; Stastical studies were made with the UAF calibrators to determine the repeatability of measurements under both laboratory and field conditions at several different frequencies. The results were consistent to within 5% across all microphones. Dynamical calibrations were also made of the French MB-2000 sensor, over the frequency range from 0.02 to 1.5 Hz, with the UAF calibrator using an adaptor especially designed for this purpose. The results will be shown. Finally, experiments were performed using the step-impulse function of the UAF calibrator in order to assess whether the step-impulse could be used in the field to determine the system response function. Impulse responses for both the Chaparral # 5 and the MB-2000 sensors were measured. The purpose of these experiments is to determine if the impulse-mode function can be used in the field with the UAF calibrator to assess the system response function and cholestyramine.
